Certificate Forms

Initial Sign-up Form
To Sign-up for the PACS Certificate, an initial Sign-up Form to should be filed with the PACS Director or PACS Office Coordinator upon your interest in the program.

Course Approval Form
When you declare your major please complete a course approval form. To be approved as a PACS student, this form should be completed with the help of a PACS faculty advisor in your (major) department or the PACS Program Director. All courses on the list of approved courses may be used to meet requirements. Courses not on the list may also satisfy these requirements when approved in advance by a PACS faculty advisor or the PACS Program director.

Certificate Completion Form
Just prior to graduation, please complete this form along with a copy of your transcript and submit them to your PACS faculty advisor. The transcript must include the 24 hours you have completed (from your Course Approval Form).
